General Info
Park City is a district known for its high performing students, skilled and dedicated staff, and up-to-date facilities. We aspire to provide each student an education that is as good as it gets anywhere in the nation.
Registration
Children who are at least five years of age before September 2 of the year in which admission is sought may enroll in school. In order to enroll a new student in school, the parent or guardian will need the following documents:

- Proof of residence (i.e. utility bill which shows parents' name and physical address)
- Birth certificate for elementary students
- Proof of immunizations: DTP - five doses (four if given after fourth birthday), Polio - four doses (three if given after fourth birthday), MMR - two doses, Hepatitis B - three, Varicella - one or history of chicken pox, Hepatitis A - two. By law, a student may NOT be enrolled in school until immunizations are up-to-date.
- Name, address, and telephone number of previous school
- If you are not the birth parent, documents showing that you are the student's legal guardian. In the case of divorce, documents showing that you are the custodial or joint custodial parent.

Transportation
Elementary students (kindergarten through grade six) are eligible for district transportation if they live one and one-half miles or more from the school that they are assigned to attend. Secondary students (grades seven-twelve) are eligible if they live two miles or more from their assigned school.

For more information regarding eligible or ineligible transportation, please contact the Park City School District Transportation Department at 645-5660 (8460 Bluebird Lane). Transportation schedules will be available at each school in mid-August.
The yearly cost of fee-paid bus service for a student who is not eligible for transportation is $200 per year and will be limited according to the number of eligible students on a particular route and logistical concerns. Availability of transportation for ineligible students is not guaranteed and will be allowed on a first-come, first-paid, first-served basis. Approval for fee-paid service at the beginning of the year does not guarantee that the approval will be for the entire year. If students may no longer be accommodated on a particular route due to the increase of eligible riders, a refund will be given based on the service quarter. The date of payment for bus service will determine priority for continued service throughout the year. Bus service may not begin until the fee is paid. Students who are approved for the free and reduced price lunch program may apply for fee waivers for bus service, but must sign up because it still is first-come, first-served.
Medication Policy
All students must have a signed permission form from a health care provider for prescription medicine.
Elementary and middle school students may not carry prescription or non-prescription medications on their person. Students in high school may not carry prescription medication on their person. High school students may carry one day's supply of non-prescription medications for their personal use only. The sharing of any medications is prohibited.
All elementary, middle, and high school students must have a signed permission form from a health care provider for prescription medicine. All elementary and middle school students must have a signed permission form from a parent or guardian for non-prescription medicines. The medicine and form must be brought to the school office.
Dress Codes
School Board policy specifically prohibits clothing or jewelry which contains any gang-related affiliation and empowers the schools to promote discipline and safety in the schools.
Clothing should be neat, clean, and modest.
To accomplish this, each school has a local dress code; however, the following standards are generally followed at all schools: Shoes, not slippers, must be worn at all times. Hats and sunglasses are not worn in school. Clothing may not display pictures, logos, or words which promote illegal substances or substances forbidden for minors, use profanity, or are in poor taste.
Clothing should be neat, clean, and modest. Clothing may not be "extreme" (too short, too bare, too tight, too baggy, too sheer, etc.). Body jewelry is not permitted.
